Why the NHS shouldn’t pay for technology that doesn’t deliver

For years, the NHS has been asked to invest in digital tools on the promise of future benefits – often without clear evidence of outcomes or aligned incentives between suppliers and the system.

Ambient Voice Technology has huge potential to transform clinical care – reducing admin burden, reducing clinician burnout and giving clinicians more time with patients. But technology alone doesn’t deliver impact. Implementation, clinical engagement, safety and accountability matter just as much.

Through The National CLEAR Programme, we’ve seen what’s possible when those principles are put into practice. NHS organisations that have deployed CLEARnotes safely and responsibly are already seeing measurable results – including a 14% productivity improvement at East Lancashire Hospitals NHS Trust and a 25% productivity increase at The Royal Wolverhampton NHS Trust.

These are not theoretical benefits. They are real, evidenced gains – achieved by putting clinicians at the centre and embedding safety and quality from day one.

But just as important as what we deliver is how we work with the NHS.

That’s why we are actively exploring shared-revenue (gain-share) models for CLEARnotes. In simple terms, this means the NHS only pays when value is proven. Our success becomes directly linked to the outcomes we help deliver – productivity, efficiency and sustainability.

This approach flips the traditional supplier model on its head. It reduces upfront financial risk, protects public money, and creates true partnership rather than transactional procurement. If the technology doesn’t deliver measurable impact, the NHS shouldn’t be footing the bill.

Looking ahead, we are keen to work with NHS organisations interested in piloting CLEARnotes free of charge for six months. These pilots will span acute, community and mental health settings, building on the learning from deployments to date. A core part of this work will be understanding whether – and how – gain-share agreements could be established following successful pilots.

For me, this is about more than a commercial model. It’s about trust, transparency and accountability. If we believe in the impact of our technology, we should be willing to stand behind it.
