Ensuring accuracy, safety, and trust
Across 6,000 CLEARnotes consultations, over 99% of AI-generated summaries were rated as semantically identical to clinician-approved notes — providing reassurance that clinical meaning and detail were preserved. No patient safety incidents were reported. In fact, the tool improved consistency and standardisation of records across the service.
Governance and information security remained central throughout the pilot. All audio data was encrypted and stored securely, with patient consent and a full Data Protection Impact Assessment completed. Continuous clinician feedback helped refine templates and vocabulary, ensuring safe and compliant use in live settings.
Enhancing clinician well-being and patient experience
By removing administrative burden, CLEARnotes enabled clinicians to spend more meaningful time with patients. Staff reported reduced stress and greater job satisfaction. As one ELHT lead commented, “CLEARnotes helped us reclaim valuable clinical time, reduce stress, and ultimately deliver better care to our patients.”
Patients, in turn, noticed the difference. They felt clinicians were more attentive and appreciated quicker follow-up communications and more accurate documentation — improving trust, communication, and continuity of care.
In late 2024, The Royal Wolverhampton NHS Trust (RWT) piloted CLEARnotes within its perioperative (pre-anaesthetic assessment) clinics. Supported by an NHS England–funded programme, the pilot set out to ensure the safe use of AVT, assess its impact on productivity and patient experience, and build an evidence base for wider adoption across the Trust.
Seamless integration and clinician confidence
CLEARnotes was integrated smoothly into existing clinical workflows, requiring no major changes to processes or systems. Clinicians retained full oversight of all generated notes and rapidly gained confidence in the AI’s accuracy and reliability. User feedback was overwhelmingly positive, with staff reporting improved patient interaction and less distraction during consultations. As one consultant anaesthetist noted, “CLEARnotes made a real difference to both my workflow and patient interactions… allowing me to focus more on listening to my patients and tailor care more thoughtfully.”
Productivity and efficiency gains
Quantitative results from the pilot were striking. Clinics using CLEARnotes achieved an approximate 25% uplift in patients processed in one clinic, meaning more patients were cleared for surgery sooner and elective backlogs could be reduced.
Documentation time was also dramatically reduced. By transcribing consultation dialogue in real time, CLEARnotes cut up to 75% of paperwork per consultation. The AI-generated summary appeared within around 10 seconds of each appointment, needing only minimal edits. As a result, documentation was typically complete before the next patient — eliminating the need for end-of-day admin or outsourced transcription.
Accelerated communication and better continuity of care
Letter turnaround improved significantly. What previously took five to seven days via manual typing was now completed and uploaded within one to two days, ensuring faster information flow to surgical teams and GPs. This rapid communication supported more timely surgical planning and enhanced continuity of care.
Improved staff well-being and patient experience
Clinicians reported finishing clinics on time, reduced stress, and greater professional satisfaction. Freed from typing, they were able to engage more meaningfully with patients — who, in turn, described feeling more listened to and better understood.
RWT’s successful pilot confirmed that CLEARnotes can be safely and effectively deployed in live clinical environments, delivering measurable efficiency gains, faster communication, and improved staff and patient experience. The Trust now plans to leverage its early-adopter expertise to scale AVT across more departments, realising further system-wide benefits.
