Conversations at the end of life are among the most important interactions in healthcare.
They are moments that require compassion, presence, careful listening and time — helping patients, families and carers feel heard, supported and involved in decisions about their care.
Yet clinicians are increasingly balancing these sensitive conversations with growing documentation and administrative demands.
What if technology could help give some of that time back?
The National CLEAR Programme is inviting hospices across England to express interest in a fully funded opportunity to deploy CLEARnotes, an AI-powered clinical documentation solution designed specifically for healthcare settings.
Hosted by East Lancashire Hospitals NHS Trust, the programme will support selected hospices to implement and evaluate CLEARnotes in both inpatient and community end-of-life care services.
What’s included?
Participating hospices will receive:
- Free deployment of CLEARnotes
- Six months of fully funded use
- Up to 30 user licences per organisation
- Structured implementation, training and governance support
- Access to a clinically led NHS-hosted transformation programme
- Support with benefits measurement and impact evaluation
- Assistance in developing an evidence-based business case for future adoption
There is no cost to participating organisations during the programme.
Supporting better care through better documentation
CLEARnotes uses ambient AI technology to capture and structure clinical conversations into high-quality documentation, helping clinicians spend less time typing and more time focused on patients and families.
The technology can support documentation across a range of hospice and end-of-life care interactions, including:
- Admissions and assessments
- Family meetings
- Advance care planning discussions
- Multidisciplinary team meetings
- Follow-up and review conversations
By reducing administrative burden and improving documentation quality, CLEARnotes can help create more capacity for compassionate, person-centred care.
More than a technology pilot
This is not simply a software trial.
The programme has been designed as a clinically led transformation initiative focused on safe adoption, measurable impact and real-world learning within hospice and end-of-life care settings.
Participating organisations will be supported to understand the benefits achieved through the programme, including impacts on clinical time, documentation quality and staff experience. This evidence will be used to develop a robust business case to inform future investment and procurement decisions.
Following the six-month programme, organisations will have the opportunity to explore sustainable models for ongoing use, including approaches linked to demonstrated value and outcomes.
